Complex surgery planning / monitoring in hospitals

Software-supported surgery planning takes a central position in many hospitals. In recent years, more and more central operating theatre areas have been formed that are used jointly by several disciplines. This requires coordinated planning, starting with a division of the operating theatre capacity into contingents, through adapted staff planning to monitoring the current plan with regard to delays and inserted emergencies.

Always a clear view with the ORSOFT surgery planning software

With a planning horizon of several weeks ahead, it makes little sense to schedule an operation to the minute. Imponderables in advance are significant and the specific operation is often too imprecisely specified. Many hospitals therefore prefer to plan the operation in detail at short notice (e.g. the day before).

Nevertheless, ORSOFT for Healthcare (ORS-H) already works with concrete appointments (as far as possible) during rough planning. In this way, realistic multi-resource planning is possible with attention to the chronological relationships between appointments and nursing activities. The “rough planner” (e.g. admission coordinator) should be aware that the planned appointment is usually correct with regard to the day, but that the time which has been determined is only provisional.

Furthermore, ORSOFT for Healthcare (ORS-H) allows to plan desired appointments like real appointments. By simple double-clicking, desired appointment can be confirmed.

Status monitor as a central tool of the surgery planning software

At the heart of the surgery planning module is an OR status monitor that shows the current status of surgery planning on the basis of the OR appointments (for operations that have not yet started) and time stamps that have already been reported back (for operations that have already started or ended). ORSOFT for Healthcare (ORS-H) works exclusively with the OR time stamps or OR phases preconfigured in Cerner i.s.h.med®. The display can be a bar chart or a calendar. The displayed time range can be freely selected, zooming is possible, as well as displaying a time range over midnight.

The status of a surgery is stored as a configurable colour scheme, surgery phases can be displayed as bars in the bar (“Matryoshka bar”). For started operations, the planned duration is projected into the plan as a frame bar in addition to the operation phases. If the planned duration is exceeded, the surgery bar is extended to the now time.

Planned operations are initially at the time for which they were scheduled. Started operations are located according to the confirmed times. In this way, an overlay of planned and actual status is obtained: As long as the operation has not been started, the plan applies; with the start of the operation, the surgery time markers count. The OR status monitor can also be delivered in a variant in which operations that have not yet been started are lined up at the now time and thus represent a queue.

The operating theatre status monitor is available in two versions:

  • as a pure display monitor, if necessary with rolling display of the operating theatres,
  • as a status monitor with integrated planning functionalities.

Extensive planning functionalities also allow

  • to postpone operations that have not yet been started, as desired, or time and space in the plan;
  • to create new operating theatre appointments and link them to clinical orders;
  • to report back and correct time stamps.

Numerous configurable icons and alarms show the patient in the context of resource occupancy, e.g. if delays to the next scheduled operation threaten to occur, and in the context of his own appointment situation, e.g. if his scheduled operation appointment now overlaps with other appointments due to postponements on the operating day. The software supports the occupancy planning and logistics of the operating theatre. Medical aspects are only recorded as documentation.
